Annual Report to Presbytery

Ministers Report Form
Our Book of Government requires the Ministerial Committee to report to the Presbytery, in our case at our April meeting, covering all aspects and categories (outside of bounds, retired, etc.,) of our membership. Take a few minutes and complete this form in order that we might update your records and report to the Presbytery.

In the areas where a longer response is required, the box will expand to accommodate your response. Just click on the appropriate check boxes and insert an “X”. If, as indicated, you need to complete your response on the back, you will need to print out the form and continue your response on the back. Upon completion of the form, print it out, sign, date it, and forward it to:
